This finding raised the suspicion of an upper gastrointestinal bleeding, which can have serious implications for his health.

During the visit, I carefully assessed his vital signs and performed a physical examination. His lethargy and overall weakness suggested a possible anemia resulting from the ongoing internal bleeding. I discussed these findings with Mr. Tan's family and emphasized the urgency of seeking immediate medical attention at the hospital emergency department.
